  1. What type of programs and activities are available to Holy Angels’ clients?
    1. Holy Angels provides care for individuals ranging in age from birth to more than seventy years of age. The programs available are built to achieve our mission to offer a path to self-fulfillment to these individuals through education, empowerment, spirituality, independence and work.   2. Who oversees and manages the clients overall wellbeing, routine and daily life in efforts to maximize their independence?
    1. If you have any questions or concerns, the very best way to communicate your needs is through the QIDP (Qualified Intellectual Disabilities Professional) assigned to your loved one. The “Q” is similar to a case manager in that he or she serves as an advocate and liaison for the resident and also ensures that the services outlined in the Individual Support Plan are correctly implemented and are consistent with the client’s wishes, needs, and desires. The QIDP provides habilitation management services and collects the information necessary to ensure a client is continually receiving informal and formal training to assist them in progressing toward their goals.
  3. Who is the QIDP or Case manager for my loved one?
  4. If I have medical questions about my loved one, who is the best person to contact?
    1. Please call the RN assigned to your loved one. We have RNs and LPNs assigned to each residential location. The assignments are as follows:

  5. I want to schedule flight arrangement for my loved one, who should I contact?
    1. Please contact our Social Services Coordinator, Judy Roge’ at 318-797-8500 ext.108
  6. How can I visit my loved one?
    1. To visit your loved one, you must be on the approved visitation list, which is provided by the guardian of the client. This list is managed by Judy Roge’, Social Services Coordinator at Holy Angels. She can be reached at jroge@laholyangels.org and at 318-797-8500 ext. 108. All visitors must check in at the front desk of the Administration building or at the residential location of your loved one.
  7. How much notice is needed when scheduling to pick and drop off up your loved one from Holy Angels?
    1. We ask for a 24 hour notice to be given to the QIDP and staff, before picking up your loved one.
  8. What if I pick my loved one up from his or her group home; what is the process?
    1. If you are picking up your loved one from his or her group home or dorm, please be sure to sign him or her out at that location. Per staff instruction, you will pick up medications at the group home or the nurses’ station from Sandra Mitchell, RN, in Madonna Hall or from Julie Campbell, RN, in the Nursery. See question #4.
  9. What if I pick my loved one up from his or her dorm; what is the process?
    1. If you are picking up your loved one from the one of the dorms, please be sure to sign him or her out at the location. If your loved one has medication, you will need go pick it up at the nurses’ station from Sandra Mitchell, RN, in Madonna Hall or from Julie Campbell, RN, in the Nursery. See question #4.
  10. What if I pick my loved one up from AngelWorks Day Program; what is the process?
    1. If you are picking up your loved one from the AngelWorks Day Program, please be sure to sign him or her out at the receptionist desk in the Administration building.  If your loved one has medication you will need go pick it up at the nurse’s station in from Sandra Mitchell, RN, in Madonna Hall or from Julie Campbell, RN, in the Nursery. See question #4.
  11. What are the procedures of dropping my loved one back off at AngelWorks Day Program or Holy Angels Residential Location?
    1. If you are dropping your loved one back off to attend the AngelWorks Day Program, please be sure to sign them in at the receptionist desk in the Administration building. If your loved one had medication please be sure to check it back in to the CMA (Certified Medication Administrator) or the nurses station at either Madonna Hall or the Nursery.

      If you are dropping off your loved one at a group home, please be sure to sign them in at their group home and return medications to the CMA (Certified Medication Administrator) in that home.

      If you are dropping off your loved one at a dorm, please be sure to sign them in at their dorm and return medications to the nurse’s station to Sandra Mitchell, RN, in Madonna Hall or Julie Campbell, RN, in the Nursery. See question #4.

  12. How much money does my loved one have in his or her account?
    1. Please call Kay Archer in the Accounting Department for this information at 318-797-8500 ext. 250.  You may also call the assigned QIDP.
  13. What if I suspect abuse and/or neglect? What should I do?
    1. Holy Angels has a zero tolerance for abuse and/or neglect of our clients. If you suspect any form of abuse and/or neglect, please call our Social Services Coordinator, Judy Roge’ at 318-797-8500 ext. 108.
